Our Team

What is Collaborative Care?

Collaborative Care is a forward-thinking approach that has been used by the physicians at the Queen Street Medical Centre for several years and makes
their practice unique.

Collaborating means organizing our health care team to have the greatest positive impact on patient outcome, using a range of professionals whose expertise is readily available to patients of the family physicians at Queen Street Medical Centre. Our goal is for patients to be seen by the right person, in the right place, at the right time.

In our clinic, we have our own on-site diabetic care team consisting of diabetic educators and dieticians who, after the initial diagnosis and treatment plan made with the doctor, work with the patient to monitor and adjust treatment on a continuing basis.

We also have a team of mental health counselors, who are available to work with patients to find effective and workable solutions to their needs.

Our team uses their specific knowledge, training, and skills to work directly with patients, ensuring timely and effective access to these important services.
