What is Collaborative Care?
Collaborative Care is a forward-thinking approach that has been used by the physicians at the Queen Street Medical Centre for several years and makes
their practice unique.
Collaborating means organizing our health care team to have the greatest positive impact on patient outcome, using a range of professionals whose expertise is readily available to patients of the family physicians at Queen Street Medical Centre. Our goal is for patients to be seen by the right person, in the right place, at the right time.
We also have a team of mental health counselors, who are available to work with patients to find effective and workable solutions to their needs.
Our team uses their specific knowledge, training, and skills to work directly with patients, ensuring timely and effective access to these important services.

Alfreda
Alfreda has been with our team as Allied Health receptionist since 2019. She is invaluable to keeping our busy practice flowing smoothly, booking your appointments, answering questions, and keeping things organized.
Laurel, LPN
Detailed nursing assessments, prenatal care, well-baby checks, bloodwork, injections, additional services.
Nancy Kane RN BScN; CPMHN(C)
A Registered Nurse with a specialty in Psychiatry and Mental Health. Nancy has worked extensively with adults diagnosed with depression, anxiety, addictions, mood disorders, and PTSD. She is certified in Mindfulness, Dialectical Behavioural Therapy (DBT), Cognitive Behavioural Therapy (CBT), Solution Focused Therapy, Strength Based Resilience Training and Cognitive Processing Therapy into her practice.
Michelle, RN
Specialized Diabetes Education.
Trudy, RD, CDE
Trudy has been a Registered Dietician and Certified Diabetes Educator for 15 years. She provides expertise in diabetes prevention and diabetes management, as well as weight, lipid, and blood pressure management through dietary intervention.
Donna, MSW, RSW
40 years experience providing tailored service working with children, youth, and families from a child welfare, addictions and mental health perspective, including those with concurrent disorders and history of trauma. Trained in Dialectical Behaviour Therapy and provides psycho-educational sessions on grief and coping skills.
Colleen Mahar, RSW, CT, CCC
As a registered Social Worker and Counselling Therapist, Colleen provides mental health support including one on one counselling, psychoeducation, and trauma informal therapy. Issues related to anxiety, depression, grief, stress, and family circumstances are met with non-judgemental support.
Darla Smith, RN, BN, CPMHN
Canadian Certified Psychiatric and Mental Health Nurse with 33 years of experience. Darla works with adults providing assistance with depression, anxiety adjustment disorders, concurrent disorders, and grief. Darla also provides assistance with Dialectical Behaviour Therapy Skills and Coping Skills.
